Current Phase
Concordia Men's Health is currently collecting applications for a future Founding 100 cohort. The application form collects contact information, state, broad interest, non-medical intent fields, consent records, and attribution data. The application form does not collect medical history, payment information, or clinical intake information.
Information We Collect
- First name, email address, mobile phone number, and state.
- Broad interest and non-medical intent fields submitted through the application.
- Consent timestamp, consent language version, source URL, page URL, and attribution fields.
- UTM parameters, campaign identifiers, ad set identifiers, ad identifiers, creative identifiers, and bridge identifiers when present.
Information We Do Not Collect In Phase 0
The Phase 0 application is not a medical intake. Do not submit medical history, symptoms, lab values, medications, diagnosis details, payment information, insurance information, body metrics, photos, or patient-specific notes through the application.
How We Use Information
- To review demand for a future men's metabolic and hormone optimization clinic.
- To contact applicants about Founding 100 application updates, content, and future enrollment information.
- To measure source, campaign, lane, and application quality.
- To keep consent and attribution records for compliance and operational proof.
Service Providers
We may use service providers for application capture, email and SMS capture, hosting, analytics, and advertising attribution. These providers may include Carrd, Omnisend, Cloudflare, analytics providers after approval, and advertising platforms after approval.
